These come out of the lane that compares each new document against every document already on record from that source. Each one is recorded as a review signal with its source, never as a fraud accusation.
Cross-vendor reuse
The same lot, report ID, file, or visual fingerprint appearing across indexed records that should not share them. The fingerprints are derived at intake, so the comparison runs on every document without anyone asking for it.
Signature and stamp reuse
Approval-signature and stamp regions compared when files carry usable visual fingerprints. A comparison that could not run is reported as could-not-verify, never as clean.
Edited outside a lab
COAs last touched by Word or a PDF editor rather than a LIMS, or created after their own stated test date. The lane reads the PDF's own metadata, and reads not-applicable when the upload is not a PDF at all.
Live FDA recall match
Recent FDA enforcement records are scanned for lot numbers known to the system, matched only on a token boundary so 1305 never matches inside 213055. A hit flags that lot in every account that logged it and stamps every passport minted from it.
Dead verify routes
Published verification links that do not resolve, or resolve to something other than the lot. The issuing-lab lane stays could-not-verify until the lab's own recorded reply resolves the document.
Recall propagation
A lot-level signal propagates only to the orders and Passports that share the same chain.
